Please print these forms, complete the information with signature and return to the Missing Child Center-Hawaii either by fax to (808) 586-1424 or mail to Missing Child Center-Hawaii, Department of the Attorney General, 425 Queen Street, Honolulu, HI 96813. Please remember to include a photo of your child. Photos will be scanned and returned to you. Digital photos can be emailed to hawaiimissingkids@hawaii.gov.

Family Abduction- Intake Forms
Family Abduction- Also called parent kidnapping or child snatching- the taking, keeping or concealing of a child or children by a parent, other family member, person acting on behalf of the parent or family member that deprives another individual of his or her custody or visitation rights. Family abductions can occur before or after a court issues a custody determination. If your missing child case is a Family Abduction, please complete the following forms and return them to our center. Non-Family Abduction- Intake Forms
Non-Family Abduction- Involves the movement of a child using physical force or threat, the detention of a child in a place of isolation, or the luring of a child younger than 15 years old for the purposes of ransom, concealment or intent to keep permanently. If your missing child case is a Non-Family Abduction, please complete the following forms and return them to our center.
