Candidate Committees - Forms

• Application For Electronic Filing Password - Candidate Committee (Writable form)
  The Application must be filed to obtain a username and password for logging into the CFS.  The form is also used to report a change in the Chairperson or Treasurer. (Revised 02/09)
• Affidavit To Voluntarily Agree With Campaign Expenditure Limits (Form CC-2) (Writable form)
  The Affidavit must be filed no later than the candidate’s filing of nomination papers with the chief election officer or county clerk.  The benefits of filing this form are: (1) Discounted filing fee, (2) Contributors may take a tax deduction, (3) Eligibility for public funds. (Revised 12/08)
• Statement Of Intent To Seek Public Funds (Form CC-4) (Writable form)
  The Statement must be filed before contributions can be considered qualifying campaign contributions for the purpose of receiving matching public funds.  There is no specific deadline for filing the Statement. (Revised 12/08)

Note: The Affidavit (Form CC-2) and the Statement of Intent (Form CC-4) must both be filed if the candidate intends to apply for public funds.  Refer to the Public Funding Guidebook Candidate Committees for information regarding qualifying and applying for public funds.

• Application For Public Funds (Form CC-6) (Writable form)
  The Application is used to apply for public funds.  The Application must be notarized and must be filed with the Commission no later than thirty days after the general election. (Revised 11/97)
• Notice Of Intent To Hold A Fundraiser (Form CC-8) (Writable form)
  The Notice is used to notify the Commission of a candidate's intent to hold a fundraiser for which the price or suggested contribution for attending the function is more than $25.  The form must be filed with the Commission prior to the date of the function. (Revised 10/09)
• Request For Registration Termination (Term Form) (Writable form)
  The Termination form is used to terminate registration with the Commission. (Revised 5/99)
  A closing bank statement for each account must also be mailed or delivered to the Commission to verify that all relevant bank accounts have been closed.
