Can I send my document for filing by fax?

We accept fax filings. Fax your document to (808) 586-2733. Legible photocopied or faxed documents will be accepted by the division. Document must be 8-1/2 x 11 inches and if signatures are not plainly legible, the name must be typed below each signature. Payment shall be made by Visa or Mastercard. The following information must be provided: Type of Card, Name of Cardholder, Expiration Date, V Code, amount of charge, expedited or regular review and a contact name and telephone number for questions.
