Instructions for Course Completion Certificate

Within 15 days of completing a course, the attendee shall receive a course completion certificate. The certificate should contain the following information, at minimum:

  • Attendee's name, vendor and license number
  • Provider name and number
  • Course title and course code (as approved)
  • Number of credit hours
  • Date of course completed
  • Date and Signature for Authorized Provider
  • Date and Signature of Attendee
