Service Contract Provider

A complete application for a Service Contract Provider registration includes:

1. FORM SCP - one original;
2. Provide the identities of the provider's executive officer or officers directly responsible for the provider's service contract business;
3. Provide the name, address and telephone number of any administrators designated by the provider to be responsible for the administration of service contracts in Hawaii;
4. Copy of each Service Contract Form provider proposes to use in Hawaii;
5. FORM FRS - one original;

6. Applicable license fees.

   Refer to FEE SCHEDULE for "Insurer, Service Contract Provider, Vehicle Protection Product Warrantor".

   Check payable to the DEPARTMENT OF COMMERCE & CONSUMER AFFAIRS.

IMPORTANT: All of the items listed above must be submitted together as an application packet. Incomplete applications will be returned to you.
