The following forms shall be submitted to coordinating agency when applying:

Adopt-A-Highway Renewal Application (for renewal applications only)

Release Form for Adopt-A-Highway Participants (if applicable)
If the application is accepted, the following forms shall be submitted:

Supply Request Form: a week before clean up (Oahu District).

Clean Up Report: within a week after clean up.

Request For Rubbish Bag Removal: within a week after clean up.
