How to Use Financial Disclosure Long Form, Short Form, and Candidate's Form

These forms can be completed by hand or on your computer. BY HAND: (1) Print out a blank form from your computer; (2) fill in the blanks of the form by hand or by typewriter; and (3) sign and date the form. ON YOUR COMPUTER: (1) Fill in the blanks of the form on your computer; (2) print out the completed form; and (3) sign and date the form. ELECTRONIC FILING IS NOT AVAILABLE AT THIS TIME, SO YOU MUST FILE A HARD COPY OF YOUR COMPLETED FORM WITH OUR OFFICE.

Candidate's Form (Form D-301)

To be filed by candidates for state elective office with the State Ethics Commission AFTER nomination papers are filed and NO LATER than 20 days PRIOR to the date of the primary election, or 20 days PRIOR to the date of the general election for candidates for the Office of Hawaiian Affairs. As a convenience for candidates, the candidate's disclosure form is included by the Office of Elections in the packet of materials together with nomination papers. The form is provided here for informational purposes or if needed by a candidate.
