BY HAND: (1) Print out a blank form from your computer; (2) fill in the blanks of the form by hand or by typewriter; and (3) sign and date the form. ON YOUR COMPUTER: (1) Fill in the blanks of the form on your computer; (2) print out the completed form; and (3) sign and date the form. ELECTRONIC FILING IS NOT AVAILABLE AT THIS TIME, SO YOU MUST FILE A HARD COPY OF YOUR COMPLETED FORM WITH OUR OFFICE.
