Are you at risk?




Know the warning signs of Diabetes!

Place an (x) beside all statements that are true for you.

A. I have been experiencing one or more of the following on a regular basis:

___   Excessive Thirst

___   Unexplained weight loss

___   Frequent urination

___   Extreme Fatigue

___   Blurry Vision


These symptoms are common to many health problems, including DIABETES, and are a cause for concern. If you have checked one or more of these, contact your physician.

B. These characteristics apply to me:

___   I have a parent with diabetes.

___   I have a brother or sister with diabetes

___   My weight is 20 percent or more above normal

___   I am of Hawaiian, Chinese, Japanese, Filipino, Korean,
         Hispanic, African-American, Native American or
         Pacific Islander descent.

___   I am over 40 years old.

___   I am not aware of having diabetes, but have been told
         before (by my physician) that my blood sugar levels are not
         completely normal.

___   I have high blood pressure.

___   I have high cholesterol levels.

___   I have had a baby weighing over 9 pounds at birth,
         or previously had gestational diabetes (diabetes during my
         pregnancy only).

If one or more of these is true for you, you are at greater risk for diabetes and should see your physician for a diabetes screening test.
