DOH Cites Chevron USA for Hazardous Waste Violations

The Department of Health (DOH) has filed a complaint and order against Chevron USA Inc. Hawaiian Refinery for alleged violations of the state’s hazardous waste rules. The alleged violations occurred at 91-480 Malakole Street Kapolei, Hawaii.

The DOH conducted routine hazardous waste inspections on March 22 and April 3, 2001. Based on these inspections, the DOH cited Chevron Refinery with two counts:

  1. failure to make a hazardous waste determination and treating hazardous waste without a permit and
  2. failure to properly complete the biennial report.

DOH has assessed a penalty of $37,000 against Chevron and has ordered Chevron to comply with State hazardous waste rules for cited violations according to a schedule established by the State.

Chevron has twenty days to respond to the order.
