How to Renew Your Placard by Mail


 You may renew your placard by following these simple steps.
1. Download application form.


2. Complete side 1.
3. Make a copy of your current valid ID.
4. Create an envelope addressed to:
  Disability and Communication Access Board
  919 Ala Moana Blvd., Room 101
  Honolulu, HI 96814
5. Take completed application, the copy of your current ID and the stamped addressed envelope to your physician.
6. Have your physician fill out side 2 of application. Sign and date item 13.
7. Either wait for your physician to complete application form and mail with attachment in your prepared envelope, or
provide attachment and prepared envelope for your physician's office to mail for you.
8. When you receive your new placard, please return your expired placard.
 To ensure receipt of your renewal on time, the completed application must be received by the Disability and Communication Access Board (DCAB) no earlier than 60 days before and no later than 10 days prior to the expiration date. Please understand that the new placard will reflect an expiration date dependant on month of issue.
