Loan Process:

Equipment will be loaned to qualified DOH professional staff servicing individuals w/ mental retardation/ developmental disabilities (MR/DD) or neurotrauma (NT) disabilities who are in need of an alternative/ augmentative communication (AAC) assessment or who are assessed as needing a short-term loan of an AAC system.  Familes, participating as community support group members to individuals with MR/DD or NT are also welcome to use the loan bank library for the same purposes, when approved on the individual service plan.  Lastly, consideration is given to private agencies servicing clients with AAC needs, other State agencies and professionals working with individuals with disabilities.

If equipment is not currently available, the borrower can consider being placed on an equipment waitlist.  They will be called pending its availability.

General Conditions of Loan Agreement:

  1. Borrower will arrange to pick up and return equipment.  Equipment is to be returned to our office on the loan agreement's specified return date.
  2. Postage will be paid by our office to send equipment to neighbor islands.  Neighbor island borrowers will pay for return postage and insurance.
  3. Equipment will be charged a maintenance/repair fee if equipment is returned in damaged condition as a result of negligence.  Charge will be determined by cost of repair or replacement.
  4. Borrower will be charged an appropriate fee for missing equipment manual or instructions.
  5. Borrower will be charged for cost or replacing equipment if lost.
  6. Loan period is for 33 working days (approximately 1.5 months).
