Submittal requirements to respond to DOH-CWB NOI comments:

A. Transmittal letter with applicable CWB File No.; response to DOH-CWB comments; certification statement; and
     original signature from Duly Authorized Representative or Certifying Person original signature.

B. CD or DVD with revised CWB NOI General Form (in pdf format); revised General Permit Specific CWB NOI
     Form B, D, E, F, G, H, I, K, L, or certified CWB SSCBMP Plan Template (in pdf format); and revised attachments (in pdf format).
     All forms and attachments shall be resubmitted in their entirety even if no revisions were made.
