Initial NOI Submittal Requirements

For coverage under a specific NPDES General Permit, the following items are required to be submitted to the DOH-CWB:

	A.	Transmittal letter.
	B.	Page 4 of the CWB NOI General Form (CWBNOI_General.pdf) with Certifying Person’s original signature [hard copy]
	C.	The completed CWB NOI General Form in pdf on CD or DVD.
	D.	The completed General Permit Specific CWB NOI Form B, D, E, F, G, H, I, K, or L (CWBNOI_B.pdf through CWBNOI_L.doc) in pdf format on CD or DVD, or a certified and complete CWB SSCBMP Plan Template (CWB SSCBMP Plan Template.doc) in pdf format on CD or DVD.
	E.	All applicable attachments in pdf format on CD or DVD.
	F.	A check for the $500 Filing Fee. Please make checks payable to “State of Hawaii”.
