NPDES General Permit Coverage Notice of Intent (NOI) Forms

A NOI is a form used to notify the Director of Health that a person seeks coverage under an NPDES general permit. A NOI consist of the CWB NOI General Form (CWBNOI_General.pdf) and the General Permit Specific CWB NOI Form B, C, D, E, F, G, H, I, K, or L (CWBNOI_B.pdf through CWBNOI_L.doc).
