Industrial Storm Water General Permittees may submit the CWB-No Exposure Certification (NOXP) Form if they believe
that their industrial activities are not exposed to any storm water.
If the subject activities are no longer occurring, the General Permittee shall submit a CWB-Notice of Cessation
(NOC) Form.
The CWB-NOI Forms, guidelines and CWB-NOC Form have been updated.
The use of
Adobe Reader 8.0 will allow you to save some of your forms in pdf format.
For coverage under a specific NPDES General Permit, the following items are
required to be submitted to the Clean Water Branch (CWB):
A. CWB NOI General Form (CWBNOI_General.pdf) with Certifying Person’s
original signature [via “Submit via Email” button and hard copy]
B. General Permit Specific CWB NOI Form B, C, D, E, F, G, H, I, K, or L (CWBNOI_B.pdf through CWBNOI_L.doc)
[via “Submit via Email” button, as applicable, and hard copy]
C. All applicable attachments [via hard copy]
D. $500 Filing Fee [Check made payable to “State of Hawaii”]
E. Additional copies as required for Islands other than Oahu [see Notes V.D.
and V.E. of the General Guidelines]
Classification of the receiving State water may be obtained from the
Water Quality Standards Maps used
in conjunction with HAR, Chapter 11-54, Water Quality
Standards or by contacting the Clean Water Branch.
Apply for NPDES General Permit coverage with the Notice of Intent
(NOI) forms and guidelines (revised 08/01/2007, unless otherwise noted)
listed below. They may be downloaded as Adobe Acrobat and/or MSWord files.
Please use these updated forms to comply with NGPC conditions.
Terminate the NPDES General Permit coverage with the Notice of Cessation form
(CWB NOC Form).

2. Insert the required information
3. Save the completed form
4. Submit via the “Submit by Email” button. Please insert the existing NGPC File No. or New Project Name in the subject line (remove the text within the parentheses).
5. Print with “Print Form” button
6. Sign (submit form with original signature)
7. Submit with the applicable discharge specific CWB NOI Form, attachments, and $500 Filing Fee.
