About the Hawaii Diabetes Prevention and Control Program
The Hawaii Diabetes Prevention and
Control Program (HI DPCP) has received funding from the Centers for Disease
Control and Prevention since 1987. Activities supported by the DPCP include the
following:
Surveillance. The Hawaii DPCP uses the Behavioral
Risk Factor Surveillance System (BRFSS) to determine the burden of diabetes in
the state. Surveillance activities also include the use of hospital discharge
and vital statistics data to better determine the impact of diabetes on the
residents of the state. The Hawaii DPCP publishes a burden report and a
supplemental update report that is shared with diabetes stakeholders across the
state.
Hawaii Diabetes Coalition. Two years of assessment
and strategic planning have resulted in the formation of a diabetes coalition. The HI DPCP has taken the lead in convening diabetes
stakeholders across the state to encourage collaboration and partnerships to
reduce the burden of diabetes in the state. Priorities for the diabetes health system
have been identified and are detailed in the Hawaii Diabetes Plan. The HI DPCP
will continue convening annual coalition meetings, providing the opportunity
for diabetes stakeholders to network, share lessons learned, and to get involved
in ad hoc workgroups addressing different goals in the plan.
Resource materials. The CDC
patient guide for diabetes, Take Charge of Your Diabetes, was revised by
the HI DPCP and other Pacific Basin DPCPs to create
an overview booklet of this larger version. The HI DPCP translated the booklet into
nine languages: Ilocano, Tagalog,
Vietnamese, Chinese, Chuukese, Marshallese, Samoan,
Tongan, and Hawaiian. These Take Charge
booklets as well as other NDEP materials are distributed to health care
professionals and community organizations across the state.
Quality Improvement Initiatives. Through partnership with the Hawaii
Primary Care Association, the HI DPCP
is working with the various federally funded community health centers (CHCs) to support their ongoing efforts in HRSA's Health Disparities Collaborative. Resources and
technical assistance are provided to the existing Collaborative teams as
requested. Additionally, the HI DPCP is involved in assisting other CHCs prepare to enter the Collaborative. The HI DPCP
partners with interested CHCs to accelerate
improvement of diabetes care in their center through their exposure to the
Chronic Care Model and the Improvement Model.
Hawaii State Practice Recommendations. Biennially, the Hawaii Diabetes Task Force meets to
review and revise the Hawaii State Practice Recommendations. The
recommendations are distributed to healthcare professionals throughout the
state through key partnerships with various diabetes-related organizations. A
poster version of the recommendations has been developed so health care
providers can place these in their exam rooms.
