This guide was written by the Centers for Disease Control and Prevention’s Division of Diabetes Translation, which is part of the National Center for Chronic Disease Prevention and Health Promotion (CDC).

The Hawai‘i edition was adapted for Hawai‘i residents by the Diabetes Awareness Coalition, Papa Ola Lökahi and the Hawai‘i State Diabetes Control Program of the Hawai‘i Department of Health. Significant assistance with the information on nutrition and exercise came from the Hawai‘i Department of Health’s Nutrition Branch and the Physical Activity Promotion Project, and Mauli Ola, Native Hawaiian Community Nutrition Initiative.
