Hawaii Diabetes Coalition

Two years of assessment and strategic planning have resulted in the formation of the Hawaii Diabetes Coalition. The Hawaii Diabetes Prevention and Control Program has taken the lead in convening diabetes stakeholders across the state to encourage collaboration and partnerships to reduce the burden of diabetes in the state.

Priorities for the diabetes health system have been identified and are detailed in the Hawaii Diabetes Plan, which will be unveiled in November 2005. The Hawaii Diabetes Prevention and Control Program will continue convening annual coalition meetings, providing the opportunity for diabetes stakeholders to network, share lessons learned, and to get involved in ad hoc workgroups addressing different goals in the plan.

The Hawaii Diabetes Plan reflects the vision of diabetes stakeholders statewide to reduce the burden of diabets in Hawaii.
