Achieving the Outcomes for CSHCN: Tools, Services, etc.

The Children with Special Health Needs Branch, with state, community, professional, and family partners, is working to achieve the National Core Outcomes for Children with Special Health Care Needs (CSHCN).

The Hilopa‘a Project is supporting an “integrated” approach. Hilopa‘a is a collaborative project of CSHNB, Family Voices of Hawaii, University of Hawaii/School of Medicine/Department of Pediatrics, and American Academy of Pediatrics- Hawaii Chapter representatives (see Project Goals and Activities).
