Chronic Disease Prevention & Health Promotion (CDPHP) is located within the Tobacco Settlement Project. Our mission is to promote wellness and improve the quality and years of life for Hawaii's people through effective prevention, detection and management of chronic diseases.
CDPHP is comprised of the following programs:

- • Diabetes
- • Heart Disease and Stroke
- • Obesity, Nutrition & Physical Activity
- • Tobacco Control
Chronic diseases, such as heart disease, stroke, cancer, chronic respiratory diseases and diabetes, are the leading cause of mortality in the world, representing 63% of all deaths. Chronic diseases are diseases of long duration and slow progression. These diseases are among the most prevalent, costly, and preventable of all health problems. Obesity increases the risk of developing many chronic conditions. Leading a healthy lifestyle (avoiding tobacco use, being physically active, and eating well) may greatly reduce a person's risk for developing chronic disease.
CDPHP fosters an integrative and coordinated approach through the building of a shared vision, strategies, partnerships and resources. Collectively, we work to promote health and reduce the burden of chronic disease for the people of Hawaii by empowering the community, influencing social norms and supporting and encouraging healthy lifestyle choices.
