What is Required of Health Care Providers that Participate in the VFC and Teen Vax Programs?

Federal VFC Requirements

  1. Comply with the appropriate immunization schedule, dosage, and contraindications that are established by the Advisory Committee on Immunization Practices (ACIP), unless (a) in making a medical judgment in accordance with accepted medical practice, the physician deems such compliance to be medically inappropriate; or (b) the particular requirement contradicts state law, including state laws relating to religious exemptions and other exemptions. (Note: the ACIP immunization schedule is compatible with AAP recommendations).
  2. Annual completion of the Hawaii Vaccines For Children Program (VFC) Provider Enrollment and Profile forms.
  3. Provide the most current version of all applicable Vaccine Information Statements (VIS) and maintain records in accordance with the National Childhood Vaccine Injury Act.
  4. Not charge for the cost of the vaccine.
  5. Not deny administration of a federally procured vaccine to a child due to the inability of the child’s parent/guardian/caregiver to pay an administration fee or impose a charge for the administration of the vaccine that is higher than the maximum fee established by the state.
  6. Screen and administer VFC program-purchased vaccine only to a VFC eligible child (less than or equal to 18 years of age). Eligibility is as follows:
    1. Is enrolled in Medicaid or Quest, or
    2. Has no health insurance, or
    3. Is an American Indian or Alaskan Native, or
    4. Has health insurance that does not pay for the vaccine [Underinsured children are eligible to receive VFC vaccine only at a Federally Qualified Health Center (FQHC) or Rural Health Clinic (RHC)].
  7. Maintain records of the child’s VFC eligibility status for a period of at least three years. Release of such records will be bound by the privacy protection of federal Medicaid law. If requested, make such records available to the State of Hawaii Department of Health.
  8. Comply with the Hawaii VFC Program requirements for ordering vaccines.
  9. Return all public-purchased vaccines to the Hawaii VFC Program in accordance with their instructions.

 

State VFC Requirements

  1. Use of the state’s yellow Official Lifetime Hawaii Immunization Record to record doses of vaccine administered for the patient’s own records.
  2. Account for all doses of vaccine received from the Hawaii VFC program and administered to children in the practice. This information will be necessary when completing your quarterly vaccine order forms and can be useful when completing the provider profile component of the VFC enrollment packet.
  3. Comply with the Centers for Disease Control and Prevention’s (CDC) Recommendations for Handling and Storage of Vaccines.
  4. Complete Vaccine Administration Visit Record and return all completed forms to the Hawaii Immunization Program on a regular basis (recommend weekly submission).
