In addition to meeting the immunization requirements for grades Kindergarten through 12 upon first school entry in Hawaii, all students must show evidence of having received these immunizations prior to 7^th grade attendance. Students who received these vaccines in infancy or childhood do not need to be re-immunized. However, all doses must meet the minimum age and interval requirements.

(a) Two doses of measles vaccine are required, with at least one of the two being MMR vaccine.

(b) Effective July 1, 2002.

2 doses of varicella vaccine are required if the first dose is administered on or after the 13^th birthday.

A documented history of varicella (chickenpox), signed by a U.S. licensed MD, DO, APRN, or PA, may be substituted for the varicella vaccine requirement.

All immunizations must meet the minimum ages and intervals between vaccine doses.

*Applies to all students attending 7^th grade in Hawaii.
