Care Provider Information
The Emergency Action Plan is a document developed by
the Public Health Nurse with the parent/guardian, student’s
physician, principal or designee, and other DOE staff, as identified,
to ensure that individuals at the school are aware of the
student’s diagnosed health condition and the actions necessary in
responding to the unforeseen/unexpected situation.
The Hawaii American Academy of Pediatrics-PHNB-DOE
Partnership Advisory (H-AAP-PHNB-DOE) has developed standardized
Emergency Action Plans for certain conditions for use in the public
school. The standardized emergency action plan will serve as a guide
and will include individualized information to meet the unique needs of
the student. Other Emergency Action Plans unique to the individual
child can be developed with the assistance of the public health
nurse.
