Care Provider Information
Completed forms can be handled in two ways:
- Give the completed forms to the parent or legal guardian with
instructions to have parent or legal guardian take the completed forms
to the student’s school, Attention: School Health Aide and Public
Health Nurse

- Send the completed form, with parental
consent, to the Public Health Nursing
Office, which provides service to the school the student attends. Refer
to the document on this website entitled, "Schools and Nursing Office,"
which will give you the school and coverage by the specific nursing
office.
