School Health Requirements - Brochures

Two brochures are available upon request. “Important Notice to Parents” summarizes immunization requirements for preschool and kindergarten through 12th grade. “Post-secondary School Requirements” lists immunization and tuberculosis examination requirements for students attending post-secondary schools. Please call (808) 586-8313 to request copies of the brochures.
