Our mission is to reduce the burden of chronic disease within the County of Kauai.
The goals of the CDMC program include:
- Increase the quality and years of healthy life for all Kauai's people
- Eliminate existing health disparities among ethnic groups in Kauai
- Support the development of community-driven strategies
- Health Education Resource Development-diabetes, cancer, cardiovascular disease, asthma, physical activity, nutrition, overweight and obesity, healthy again, sun protection and smoking cessation

Bilingual Health Services
The goal is improve the delivery of preventive health services and increase healthy behavior among non-English and limited English speaking residents of the state.
Bilingual health aides provide a variety of general health education and assistance to various programs of the State Department of Health in language and cultural interpretation of disease management and control to alleviate barriers in delivery of public health services to non-English speaking and limited English speaking populations on Kauai, Filipino, Hawaiian and Niihau populations are provided support in accessing services through the two bilingual health aides.
- Provides community outreach, home visits, representation on Filipino and Hawaiian agencies
- Provides family and individual counseling, screening support, follow-up, referral, and new contacts related to accessing medical and social services necessary for the client
