Licenses to perform marriages are issued by the State Department of
Health under Hawaii
Revised Statute 572-12.
Individuals interested in becoming licensed marriage performers
should read the following information carefully to determine which
procedures they must follow in order to be granted a license. Should

The Current Registration Section of the Vital Records office is
responsible for the licensing of marriage performers.
Applicants on O‘ahu should apply with a letter of
recommendation and I.D. between the hours of 8:00 a.m. and 1:30 p.m. at
the Department of Health Vital Records Office, 1250 Punchbowl St., Room
105. The applicant should go to the security guard in the lobby for
assistance. Neighbor Island applicants and out of state applicants
should contact the Department of Health at (808) 586-4540 for
application information.
If you are a minister establishing a new denomination in the State
of Hawai‘i which has no religious affiliation elsewhere and you
are the head of the church, submit a letter of recommendation from the
members of the Board of Directors or Board of Trustees (signed by each
member with his name address and telephone number typed under his
signature). In addition, submit a letter stating that you are the head
of the church in Hawai‘i and a xerox copy of the church
incorporation papers. An I.D. with a photo, such as a driver's license
or state I.D. card must be presented.
