from CDC BRFSS definition, which is based on one question, question 2.1 in section 2.
[2] Source of health insurance coverage is also called type of coverage. This type of coverage variable is derived from
questions 2.2, 2.3, 2.3a in section 2. This differs from the CDC BRFSS definition.
[3] Complete set of permanent teeth means none of the permanent teeth was removed. The word none is what
is shown in Table 6b.
