Form 1099




 
Unemployment Insurance

 

Form 1099-G will be mailed towards the end of each January to all who received unemployment insurance benefits in the previous calendar year. The form will include the amount of benefits paid and other information to meet Federal, State, and personal income tax needs for the tax year. 

 

1099 information is now available to view on-line at:  UIClaims.Hawaii.gov.   Select “Claim Inquiry” from the main menu page and then select “Display 1099 Information.”  (Note: 1099 information will be displayed after Form 1099-G has been mailed at the end of January.)

 

To ensure receipt of Form 1099-G, make sure that the Unemployment Insurance (UI) Division has your most current mailing address. To update your address or to obtain a duplicate Form 1099-G if the original was not received within a reasonable period of time, contact your local UI claims office.  Click here to obtain local claims office phone numbers and email addresses.
